2. The printout titled "Bank Statement of Account No. for the period
from 20/07/2000(1nception) to 04/02/2002 (closing date) and
issued on 18th January 2018 (attached herewith) contains
information stored in the computer used for storing customer
transaction details.

3. The said printout was produced by the computer during the
period over which the computer was used regularly to store and
process information for the purposes of activities regularly carried
on over that period by lawfully authorized persons.

4 During the said period, information of the kind contained in the
electronic record was regularly fed into the computer ih the
ordinary course of the said activities.

5. Throughout the material part of the said period, the computer
was operating properly.

6 . The information contained in the electronic record reproduces
such information fed into the computer in the ordinary course of
the said activities.

7 I am in a responsible official position in relation to the operation
